Terms & Conditions
Please carefully refer to these terms and conditions, as they are part of our contract
with the service user. These are no nonsense conditions forming what your obligations
to ourselves and what you expect from us.
1.
CONTRACT, Your contract to us is in place between Choice Care Services
and yourselves. When a booking form is filled in and deposit paid,
confirmation is sent. This indicates that the contract is in place.
2.
PROVISIONAL BOOKING. A provisional booking will be held for fourteen days from
the time of booking if a completed booking form and the correct deposit are not
received by the end of this time period the booking will be cancelled.
3.
PRICES, The prices are set at time of going to press
(these are a guide price, should clients have extra needs,
e.g One to One personal care then a supplement will be charged accordingly)
should anything change due to unforeseen circumstances then we hold the right
to vary the price, but if the booking form is in place and deposit paid we have
no right to change the price of your holiday.
4.
WHAT IS INCLUDED, Our holiday includes return travel of up to 400 miles
round trip, full board accommodation in a single room, all organised
activities to include entrance of up to 4 attractions per week of stay
and staff care and support. What is not included is souvenirs, drinks
from the bar and snacks whilst out. We do operate a guest bank system
for guests to keep their money in our safe. One to one personnel care
and one to one escort services are not included in the price, these are
charged as extra packages at very reasonable rates. There is no charge
for any of the activities or visits undertaken during the holiday.
5.
PAYMENT, The initial payment at time of booking is 50% of the total
cost of the holiday; the balance must be paid at least 4
weeks before the start of the holiday. If you have to
cancel the holiday then you are liable for the costs set out below.
Over 4 weeks = 25% of the cost of the holiday
6.
CANCELLATION OF THE HOLIDAY, If you wish to cancel the holiday after
confirmation has been sent then you must do so in writing to Choice
Care Services The cancellation will be effective when we receive the
letter and the charge will be levied by the above charges. We reserve
the right to cancel your holiday due to uncontrollable reasons. We will
either try to offer you an alternative holiday or refund you 100% of
your holiday cost.
7.
INSURANCE, Our charges do not include holiday insurance. We encourage
you to seek holiday insurance cover. We are covered for third party
insurance to 10,000,000.
8.
COMPLAINTS, If any guest has a complaint whilst on holiday please
tell a senior member of staff immediately who will be happy
to help and solve any problems. If you still feel unhappy then
please complete a complaint form and return it to a senior member
of staff or within 14 days to the office address. If the complaint
involves issues under the Registration of Homes act then a copy will
be forwarded to The Commission for Social Care Inspection. Any
complaint will be investigated and a full report sent back within
14 days of receiving the letter.
9.
BEDROOMS All bedrooms are for single occupancy.
10.
ARRIVAL & DEPARTURE, This is normally negotiated between yourselves
and our transport co-ordinator. But normally you will be collected
at around 11am and depart the facility for home around 10:30am.
11.
STAFF RATIOS & CARE PACKAGES, We will take positive steps to insure a
minimum staff ratio of 1 – 4 during the day and 1 – 6 at night. There
is always Senior Management back up on site 24hrs a day when guests
are on site. For guests requiring greater staff care either because
of high dependency and challenging behaviors a care package would be
drawn between the service user and Choice Care Services.
12.
REPORTS, All guests leave with copies of any paperwork such as
medication cards etc. If you wish to have a full in depth report to
use for reviews etc. on the guests stay then please ask, charges for
this will be £25.
13.
PERSONAL SICKNESS OR INJURY, In the unlikely event of an injury or sickness
we will take the appropriate action we deem necessary and keep key personnel
informed at all times. It must be re-iterated that guests should have appropriate
insurance to cover loss or accident whilst on holiday.
2 – 4 weeks = 50% of the cost of the holiday
1 – 14 days = 75% of the cost of the holiday
Start date = 100% of the cost of the holiday
Finally in these terms and conditions the words “you” and “your” refer the to personthat made the booking or the guest as appropriate.
